COMPENSATION AND BENEFITS

ABOUT THE JOB

We are seeking a Restorative Coordinator to assist, as directed by therapists, to ensure that quality care is being provided to each resident. Responsibilities include:

  • Coordinate and monitor the restorative programs for long term residents and short-term patients in the skilled nursing facility

  • Routinely meet with nursing leaders and DOR to report and review the residents on a restorative program(s); collaborate with nursing leaders to determine appropriateness of programs and track changes or updates to the programs

  • In partnership with the nursing facility leadership, monitor medical records to ensure restorative programs are being carried out, documented appropriately, and coded into the MDS accordingly

  • Assist in the training of certified nursing assistant staff on the delivery of nursing restorative plans. Enlist support of other nursing leaders or rehabilitation professionals when needed

  • Collaborate with the rehabilitation professionals and DOR to make sure residents and short-term patients are transitioned between skilled rehabilitation and appropriate nursing restorative programs. Ensure there is not redundancy between restorative programs and rehabilitation plans of care within the medical record

  • Assist in the identification and training of weekend certified nursing assistants to carry out and document the nursing restorative programs

  • Carry out restorative nursing programs for an appropriate number of short-term patients and long-term residents, as determined by rehabilitation and nursing staff

  • Provide superior customer service to all staff and residents, maintaining an excellent working relationship with hospital staff and projecting a positive and professional image at all times

  • Adhere to established confidentiality standards

  • Demonstrate and understand the importance of and respect for the rights, dignity and individuality of each patient visitor, vendor and staff in all interactions

  • Promote the mission, ideals and values of Infinity Rehab

  • Attend in-services and staff meetings as directed

  • Perform other duties as assigned

Qualifications

Recommended Minimum Position Qualifications:

  • High School Diploma or Graduate Equivalency Degree required

  • Current Certified Nursing Assistant certification

  • One to two years of previous health care experience is required

  • Current CPR certification

  • Maintains current health standards as required by agency

Preferred Qualifications:

  • Experience in a rehabilitative setting as well as general office experience
