Position Description:
In the role of Patient Coordinator in the department of Family Medicine, you will primarily be working with specific ethnic communities. This individual must be bilingual, fluent speaker of a target refugee language (Swahili). This position will be responsible for helping refugee patients and their families to navigate and access community health services and adopt healthy behaviors. This position supports medical providers and voluntary agencies (VOLAGs) through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their family and advocate for individuals and community health. Community outreach, home visits and leading community education sessions will be required. Primary Functions Knowledge - Skills - Abilities
